Patients living in the catchment area can register with the practice. New patients are asked to complete a registration form and are expected to attend a new registration health check. This will include questions about your medical history and vaccination details. Health checks also include taking blood pressure and lifestyle advice.
Unfortunately due to strict NHS guidelines, we are unable to accept practice registrations online. However, we have provided the forms for you to download. You will need Adobe Acrobat Reader to download the file.
Simply print off the form at home, or come into the practice and we will provide you with a form. You will need to bring proof of identification (such as a passport or driving licence) and a proof of address (such as a utility bill). It is also helpful if you can give us your previous GP’s contact details so that we can arrange to get your medical details.
When register we will invite you in for a new patient health check, usually with our healthcare assistant. We are often able to carry out this check at the time of registration.
